CLARIFICATIONS & EXCLUSIONS

The preceding pricing is based on the following contingencies:

1. All work to be done during regular time working hours; Monday through Friday; 7:00 AM to 3:30 PM unless otherwise stated.

2. No floor/wall preparation, demolition, furniture moving, existing adhesive removal, cleaning or removal of oil, grease, solvents, paints, plaster or other foreign substances, shot blasting, grinding, bushing, filling, leveling, capping or skim coating, crack isolation/suppression, mud beds, scratch coats, wall blocking, or cement board is included other than as stated. Floors shall be filled, leveled, swept, scraped, free from any and all construction equipment, materials, trash, and workmen prior to arrival of CTE flooring installers. Should any additional floor prep etc. be required and authorized, it shall be done on a time and material basis at $35.00 per man per regular time hour, $16 per 25# bag Vitex Floorstone, $24 per 25# bag Vitex Ultra, $17 per gallon Latex Additive, $9 per Sanding Disk, $60 per 40# bag Ardex K-15, $44 per gallon Ardex Latex Primer P-51, $42 per gallon Emulsion Additive E-25, $64 per unit Epoxy Primer P-82, $26 per 10# bag Ardex Feather Finish, $43 per 40# bag Ardex SDP, $52 per 50# bag Vitex SFU, $27 per gallon Acrylic 620 Primer, $18.50 per package 8" scraper blades, $6 per package 4" scraper blades and $22 per box Floor Sweep. Pricing for any additional materials required will be calculated by your CTE Sales Representative as needed. Appropriate Sales Taxes Will Be Added.

3. No vacuuming after installation is included.

4. No wet mopping, buffing, waxing, or floor, wall, or countertop protection before or after installation is included.

5. No concrete sealer/primer, or tile sealer is included nor the labor to apply such sealer/primer, other than as stated.

6. Hoist and elevators must be available for free for use in stocking of floor covering materials and tools (if craning or scissor lifting is required, the expense will be covered by others). Stocking shall be scheduled so as to incur no downtime to CTE employees or subs. Downtime will be billed at $35.00 per man per regular time hour.

7. Terms: All initial materials to be paid in full in advance, before any ordering is done. Payment for all labor and any additional materials is DUE IN FULL within 5 days of completion of project. NO RETENTION SHALL BE WITHHELD FROM ANY INVOICE. INTEREST @ 2% PER MONTH WILL BE CHARGED ON ALL INVOICES BEYOND STATED TERMS, PLUS REASONABLE ATTORNEY FEES.

8. Any changes, addendum, or design configuration adjustments, which change the original blue prints the job was bid from (or scope of work stated on job walk) will be re-priced by CTE to make sure that numbers as stated will still apply.

9. Permanent lights, heat and HVAC system shall be on completely for each phase of installation. 70 degrees shall be maintained 48 hours prior, during, and for 72 hours after installation. 50 degrees shall be maintained at all times thereafter. Any violation of this contingency or failure by the General Contractor /Owner to meet such conditions shall result in loss of installation warranty immediately for all areas affected and progress of work will be stopped until conditions are rectified.

10. Pattern side match run off correction is not included in stated pricing. Bias correction, bow correction, and/or skew correction is not included. Any correction required and authorized will be done on a time and material basis at $35.00 per man per hour @ regular time working hours, plus materials.

11. Installation shall be scheduled in pre-agreed phases (established in cooperative planning between CTE and other project managers) so as to insure manpower is available and pricing as stated shall apply. Pre-set phases will be completely restricted from people traffic and other construction tradesman during that specific floor covering phase.

12. Furniture moving, construction and/or people traffic during first 24 hrs. after glue to pad method carpet installation will void installation warranty.

13.Door cutting and resizing due to the height of the floor covering, to be done by others.

14. Includes no attic stock of materials beyond installation coverage UON.

15. Warranty excludes deficiencies resulting from substrate, undetected moisture, consequential damage, and damage by others. Also not included is failure of flooring due to cracking of substrate or delamination caused by moisture or vapor pressure.

16. Per CRI-104-96, 6.3 SITE CONDITIONS: The owner or GC must submit to the flooring contractor a written report on moisture and surface alkali of the slab to determine its suitability for the materials to be installed.

17. Bonding is not included UON.

18. Abbreviation meanings: NIC = Not In Contract; UON = Unless Otherwise Noted; CTE = CT Enterprises; GC = General Contractor.

All Material is guaranteed to be as specified.

Warranty for 1 year from date of substantial completion for problems associated with installation. CT Enterprises will not accept charge backs of damage or cleaning without the option to inspect claim/s to repair, or without the option to make arrangements for acceptable repairs at their expense. CT Enterprises is not responsible for any claims that might result from product delivery date changes beyond their control. This proposal is valid for thirty (15) days. Any alteration or deviation from above specifications involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate.

WARRANTY DOES NOT INCLUDE ANY MOISTURE RELATED SUBFLOOR FAILURES
